6.  Antibiotic Stewardship Principles and IV to Oral Antibiotic Conversion

Antibiotic overuse in long-term care is common, prompting calls for antibiotic stewardship programs (ASP). IV to PO conversion is promoted as one of the many key strategies of an effective ASP that has been repeatedly demonstrated to be therapeutically effective, convenient, safe, and cost-effective yet still remains a practice that is often underutilized. This presentation is designed to provide an overview of Antibiotic Stewardship Program Basics and include considerations on expanding current programs to include IV to PO conversion. According to the CDC, improving the use of antibiotics in healthcare to protect patients and reduce the threat of antibiotic resistance is a national priority. It seems simple, but it takes the commitment of everyone involved to put it into action. The CDC also estimates that 40-75% of antibiotics are prescribed incorrectly in nursing homes, and that almost 50% may be given for a longer duration than needed. The intent of this presentation is to discuss antibiotic stewardship program goals and examine steps long-term care facilities can take to make them more successful.
